Eligibility

All applicants must demonstrate the ability to live independently and display enough emotional maturity to pursue education and/or job training.
- Adults 18-25 years of age
- At risk of or homeless
- May be a client of Sierra Wind Wellness Center
- Must be Sober
- Must sign a consent for drug and alcohol testing
Applicants must not have been convicted or a violent crime against persons or property within 2 years of application; nor an applicant can not have been a gang affiliate or be a 290PC registrant.

Transitional Housing
True Transition's new program provides young adults with the opportunity to gain confidence and skills through a mentor relationship with other young adults and experienced social workers.
Based on college dorm models, this program bridges dependency with independency. Our program is a supervised program with staff on site to assist with the day to day challenges of being an adult. |
