HWR TRUE TRANSITIONS PROGRAMS

Extending Mentoring Services to Reach Juvenile Justice and Foster Care Youth

Most of us can remember that one adult - teacher, parent, or neighbor - who made our transition from adolescence into adulthood a little easier.  But for young people in juvenile justice or foster care system, finding adults who can provide ongoing support and understand the unique challenges these youth face can be a difficult and sometimes impossible task.  And while numberous agencies help connect young people to mentors, very few are equipped to assist these at-risk youth, leaving many without the guidance they so desperately need.

Mentoring

  • Provide a stable and safe living environment
  • Provide academic support
  • Help residents set goals for education, vocation, or career
  • Help residents further develop their skills in the area of problem solving, social interaction, communication and deductive reasoning
  • Increase resident involvement in community service
  • To help each resident secure permenent housing in the community of their choice

OUR VISION

To create safe environments and educational programs that will provide the necessary support and tools to teens and young adults while they transition from dependency to independecy.
To help each resident develop life strategies that focus on enhancing their relationships, better self-esteem, and develop short and long term career goals.

Transitional Housing

True Transition's new program provides young adults with the opportunity to gain confidence and skills through a mentor relationship with other young adults and experienced social workers.

Based on college dorm models, this program bridges dependency with independency.  Our program is a supervised program with staff on site to assist with the day to day challenges of being an adult.
